Welcome to the Transition Support Programme Year Two Self Assessment Questionnaire for local areas.
To help you gather the answers you need, we have created a word version of the questionnaire. Before you complete the online questionnaire, you might want to use the word document to collect your responses. Please make sure everyone with responsibility for transition in your local authority and PCT is in agreement with the answers you will be submitting. All supporting documents can be downloaded from the self assessment area of our website.

The online version of the SAQ can be saved and returned to. The person that leads on transition in each local area should have received an email from the National Transition Support Team on Tuesday 1 September 2009 with a username and password. If you are leading on transition in your local area, and have been working with a TSP adviser from National Strategies, the Child Health and Maternity Partnership or the National Transition Support Team, but HAVE NOT received this email please contact us on:
