Welcome to the Transition Support Programme Year Three Self Assessment Questionnaire for local areas.
Please note your answers from SAQ 2 will appear in the SAQ 3 online form. However a word version of the questionnaire is also available to help you gather answers you need. Before you complete the online questionnaire, you might want to use the word document to collect your responses. Please make sure everyone with responsibility for transition in your local authority and PCT is in agreement with the answers you will be submitting. All supporting documents can be downloaded from the self assessment area of our website.
This year your answers from SAQ 2 will appear in the SAQ 3 online form. All you need to do is make any amendments to answers to reflect developments in your local area over the last 12 months. A few additional questions have been added to gather information to feed into the Green Paper on SEN and Disability and to understand the impact of any changes in local provision on transition.
The online form has the same questions as the word version. It has a mix of tick boxes and free text boxes. Information can be pasted into the Free Text boxes from Microsoft word. The online version can be saved and returned to so that you can fill it in gradually, rather than in one sitting. You can print sections of the online questionnaire before submitting so that colleagues can agree the content. To print sections use the File menu at the top left of your browser. You can also print the whole SAQ by clicking the ‘Print’ option once you have logged in. This will open a new Window, and you will then again need to use the File menu at the top left of your browser.
The person that leads on transition in each local area should have received an email from the National Transition Support Team in the week beginning 11 October 2010 with a username and password. If you are leading on transition in your local area, and have been working with a TSP Named Adviser from National Strategies, the Child Health and Maternity Partnership or the National Transition Support Team, but HAVE NOT received this email please contact us on:
